Exodus PCS provides state licensed Relocation Service Coordination (RSC-TCM) and Moving Home Minnesota (MHM) transition services. We work with eligible nursing facility residents to locate housing in the community and put supports in place as they transition into the their new home. We pride ourselves on our patient advocacy, offering Person Centered Services and ensuring smooth successful transitions.

I honestly don’t know how I would have managed this life-altering and confusing transition with my dad if we hadn’t had Alexis from Exodus. Scott from Exodus PCS was invaluable in finding a new home for my brother. The long-term care center my brother lived in closed and we were on a time crunch to find him a new home. Scott helped find a group home that specializes in traumatic brain injuries which is essential for my brother's care. Although the process was intimidating. I was assisted throughout the whole transition. Scott is knowledgeable, personable and has great compassion for the clients and community that Exodus PCS serves. Thanks to Lara at Exodus who was referred by Dakota County after really fast work by Jill O'Brian in McLeod county and my care team at M-Health Fairview Eagan. I was reunited with my beloved emotional support animal ❤ My cat Callie after at a beautiful assisted living community in Minnetonka, MN. Lara truly worked a miracle that I was praying to God for! Along with this room in this beautiful building they purchased me a new 🛌 bed, a new dresser, a new recliner, blackout blinds for when my headaches were getting worse and sheets w/pillows, a new shower curtain and towel set. All of this when I needed my cat most of all. The love and blessing In my heart from a group of people that barely knew me was enough to put this 44 Year old man in tears of Thanksgiving.
