Exodus PCS is a contracted provider with Managed Care Organizations (MCO) within Minnesota. We assist members in transitioning from nursing facilities into the community by locating appropriate housing, arranging services and discharge planning. This includes working alongside care coordinators to ensure smooth transitions and wrap around services.
