Moving Home Minnesota (MHM) is a Medical Assistance funded and federally approved program to assist individuals in moving back into the community. It’s goal is to support person’s transitioning by providing a transition coordinator and ensuring details from the transition plan are met.

Exodus PCS is also a provider of Moving Home Minnesota Transitional Services. These supports assist in several aspects of a transition including obtaining security deposits, furniture, household goods and even a moving company.
